阵发性心房颤动患者红细胞分布宽度的变化及临床意义

摘    要:目的探讨阵发性心房颤动患者红细胞分布宽度(RDW)的变化及临床意义。方法连续入选2012年3月至2013年3月于天津医科大学第二医院心脏科住院的非瓣膜性阵发性心房颤动患者133例,以及年龄、性别、合并疾病相匹配的非心房颤动患者101例作为对照组,记录两组患者年龄、性别、合并疾病、实验室指标、超声心动图结果,并比较两组各项指标的差异。结果两组间年龄、性别、合并高血压、冠心病、糖尿病等疾病情况差异均无统计学意义。阵发性心房颤动组RDW水平显著高于对照组(12.71%±0.90%比12.45%±0.62%,t=-2.457,P=0.015)。与对照组相比,阵发性心房颤动组患者左心房直径增加(38.79±7.07)mm比(34.46±4.51)mm,t=-5.405,P<0.001],左心室射血分数下降(58.90%±7.62%比61.52%±5.13%,t=2.957,P=0.003)。应用多变量Logistic回归分析显示,RDW是阵发性心房颤动的独立危险因素(OR:1.626,P=0.044)。结论阵发性心房颤动患者RDW水平明显升高,RDW可能是阵发性心房颤动发生的独立预测因素。

Abstract:Objective To investigate the changes of red blood cell distribution width (RDW) and its clinical significance in patients with paroxysmal atrial fibrillation (AF). Methods Our study enrolled 133 patients with paroxysmal AF and 101 subjects as control matched for sex, age and atheroselerotic risk factors. Sex, age, previous medical history, laboratory parameters, and echocardiography were analyzed between the patients with AF and controls. Results There were no significant differences in sex, age and comorbidities between the two groups. The level of RDW in AF group was significantly higher than that in control group (12.71% ±0.90% vs. 12.45% ±0.62%, t = -2.457, P=0.015). Compared with the control group, left atrial diameter (LAD) was increased (38.79 ±7.07) mm vs. (34. 46 ±4. 51 ) mm, t = - 5. 405, P 〈0. 001 ] and left ventricular ejection fraction (LVEF) was decreased (58. 90% ±7.62% vs. 61.52% ± 5.13% , t = 2. 957, P = 0. 003 ) in AF group. Multivariate logistic regression analysis showed that RDW was an independent predictor for paroxysmal AF ( OR : 1. 626, P = 0. 044 ). Conclusions Paroxysmal AF has higher level of RDW compared with controls, and RDW is an independent predictor for paroxysmal AF.
