联合手术治疗下肢动脉缺血性病变的实验研究

目的：探索一种治疗下肢动脉缺血性病变更有效的手术方法。方法：取健康杂种犬5只，行分期股动静脉转流术，第2期手术后2w加行腰交感神经节切除术，用彩色多普勒检测加行腰交感神经节切除术后吻合口近端股动、静脉血液动力学变化。结果：加行腰交感神经节切除术后，吻合口近端股动、静脉血流量分别增加60％和48.1％。结论：联合手术可以起协同作用，使缺血肢体获得更多动脉血，静脉血回流量亦相应增加，是治疗下肢缺血性病变更有效的一种手术方法。

Revascularization of lower extremities by staged arteriovenous reversal with lumbar sympathectomy

Objective: To explore a more effective surgical method to treat occlusive disease in peripheral artery. Methods: Five conditioned mongrel dogs (ten hindlimbs) underwent staged arte- riovenous reversal. Two weeks after the seeond stage operation, lumbar sympathectomy was per-formed. The hemodynamic changes of femoral artery and vein proximal to the arteriovenous anas-tomosis were investigated with color doppler flow imaging before and after sympathectomy. Re-sults: After sympathectomy, flow through the femoral artery was increased by 60%, flow through the femoral vein was increased by 48. 1 %. Conclusion: Staged arteriovenous reversal with ipsilater-al lumbar sympathectomy would be a good combination. It can supply more nutrient flow to an ischemic lower extremity. The venous blood flow returning to the heart would increase correspondingly.
