| 摘 要： | 本文回顾了社区健康管理世界和中国发展趋势。利用文献复习，探讨了中国社区慢性病负担和社区慢性病健康风险因素。同时，对社区健康管理实施模式和发展战略进行了可行性研究，基本内容是：①在整个国家层面形成一个健康理念；②在社区健康管理运作方面构建一个体制，即社区和综合性医院以及体检中心联合机制；③形成一个模式，就是政府搭台，社区实施，专家研究，企业参与。

| Abstract: | This text looked backed the community health management. Making use ofed the cultural heritage, we study chronic disease support in Chinese community and the chronic disease health risk factor in community. At the same time, we study community health the management development mode. Making use of the second-hand data, we make an evaluation in community health management. The basic contents are：（1）coming into being a health principle in national level;（2）seting up a system in the community health management operation, unit mechanism of community and hospital and check-up center t ;（3）becoming a mode, government to take the set, community to into practice, experts to do research, business to enterprise with. |
