急进高海拔动脉弹性功能的变化及其临床意义

摘    要:目的 探讨高海拔低氧环境对急进高海拔健康男性动脉弹性功能相关指标的影响情况及其临床意义。方法 采用美国 pulsemetric公司的肱动脉无创性动脉弹性测定仪 (型号Dyna Pulse 2 0 0M )对 3 8例平原男性正常人在海拔 5 0米、急进至海拔 40 0 0米当天的动脉弹性功能相关指标进行动态追踪观测。同时观察记录急性高原反应 (AMS)发生率。多元回归及Logistic回归分析二者间的相互关系。结果 平原健康男性急进高海拔地区 ,收缩压 (SBp)平均升高了 6.6%、系统动脉阻力 (SVR)明显降低 (P <0 .0 5 ) ;而高海拔低氧环境不影响健康男性肱动脉顺应性 (BAC) (P>0 .0 5 ) ;Logistic回归分析表明平原人急进高海拔地区SVR值变化的大小是AMS发生的独立危险因素 (F =16.0 4,P <0 .0 0 1)。结论 平原人急进高海拔存在动脉血SaO2 及SVR的持续降低 ,和一过性SBp增高 ,临床上测定SVR值变化的大小可作为筛选AMS高危人群的方法之一 ,对防治AMS具有指导意义

Abstract:Objective To investigate the relationship between hypoxia at high altitude and arterial compliance of healthy men on acute exposure to high altitude and clinical importance.Methods 38 subjects who had lived in sea-level were included in our research.All subjects underwent non-invasive office BP measurement ( Dynapulse 200M;Pulse Metric,Inc.,USA) in sea-level or in high altitude (4000m)to obtain data of the arterial compliance of healthy men on acute exposure to high altitude.Results On acute exposure to high altitude in the sea-level natives,SBp was increased to 6.6%,and SV Resistanc(SVR) decreased significantly at both altitude(P<0.05);but BA compliance(BAC) did not affected by change between both altitude(P>0.05).Logistic regression showed that significantly charged SVR was an importantly dangerous factor for AMS (F=16.04,P=0.000).Conclusions out results demonstrate the continual decreasing of SaO 2 in arterial blood and SVR were observed on acute exposure to high altitude in the sea-level natives,SBp increased temporarily.Estimate of SVR may be an important screening to high danger population of AMS in clinic.
