周围型肺癌的螺旋CT征象及病理特征与MMP-2表达的相关性研究

研究MMP-2（基质蛋白酶-2）在肿瘤浸润和转移过程中的表达,并探讨其与周围型肺癌螺旋CT征象及病理学的关系。方法：收集周围型肺癌57例的螺旋CT影像学表现及病理、MMP-2免疫组织化学资料。结果：57例周围型肺癌中,MMP-2阳性表达35例,阳性率为61.40%,MMP-2的表达与病理类型无关（P〉0.05）,与病理分级显著相关（P〈0.05）。MMP-2在肿瘤直径〉3 cm的表达强度高于肿瘤直径≤3 cm者。与肺癌CT征象中深分叶征、棘突征、纵隔淋巴结转移相关（P〈0.05）,与毛刺征、胸膜凹陷征没有相关性（P〉0.05）。周围型肺癌的强化程度与MMP-2表达有一定的相关性（P〈0.05）。结论：MMP-2的异常表达与周围型肺癌的病理分级相关,提示MMP-2在肿瘤浸润和转移中起着重要促进作用;MMP-2异常表达与螺旋CT征象中肿瘤直径的大小、深分叶征、棘突征、纵隔淋巴结转移相关,是其形成的分子生物学基础。MMP-2的表达与螺旋CT征象相关,可以在蛋白表达上判断周围型肺癌的侵袭、转移和预后。

Relevant Research between Expression of MMP-2 and SCT and Pathologic Findings in Peripheral Lung Cancer

Objective：To study the expression of MMP-2 in the process of tumor invasion and metastasis,explore the relationship between SCT and pathologic findings in peripheral lung cancer. Methods ： CT images of 57 patients with peripheral lung cancer and pathological, immunohistochemical staining of MMP-2 information were collected. Results：35 of 57 patients with peripheral lung cancer had the expression of MMP-2, the positive rate of 61.40%. The expression of MMP-2 was not relevant with histologic type （P〉0.05）. There was a significant correlation with the pathologic grade （P〈0. 05）. The expression of MMP-2 in tumor which diameter〉3 cm was more than that diameter≤3 cm and relevant with CT findings of lung cancer of deep lobulation, spinous protuberance, the mediastinal lymph node metastasis （P〈 0.05） and no correlation with spicular signs, pleural invasion （P〉0.05）. The degree of enhancement of peripheral lung cancer was associated with the expression of MMP-2（P〈0. 05）. Conclusions：The expression of MMP-2 correlates with the pathological grading of peripheral lung cancer. MMP-2 playes an important role in promotion in tumor invasion and metastasis; Abnormal expression of MMP-2 has a relevance with CT findings of tumor diameter size, deep lobulation, spinous protuberance, mediastinal lymph node metastasis and is the foundation of molecular biology. CT findings associated with the expression of MMP-2 can evaluate invasion and metastasis and prognosis of the peripheral lung cancer according to protein expression.
