胃肠道间质肿瘤70例临床病理分析

目的 探讨胃肠道间质肿瘤（gastrointestinal stromal tumors，GISTs）的临床病理及免疫组化特点。方法 收集70例GISTs的临床相关资料，观察病理组织学特征，采用HE染色及免疫组织化学方法进行研究。结果 70例GISTs患者的临床表现及影像学均类似于其他胃肠道肿瘤，病理形态学特点类似平滑肌肌源性肿瘤及神经源性肿瘤。免疫组化染色CD117阳性率为100％（70／70），CD34阳性率为75．7％（53／70），S-100阳性率为40％（40／70），Desmin，Aetin，MBP，CK均为阴性。结论 GISTs是消化道较少见肿瘤，临床病理特征与平滑肌肿瘤及神经源性肿瘤类似。免疫组织化学染色CD117，CD34标记阳性是确诊GIST最具有诊断价值的依据。其预后治疗不同于其他类型胃肠道间叶性肿瘤。

A Clinic-pathological Study on GISTs in 70 Patients

Objective To investigate the clincopathological and immunohistochemical characteristics of gastrointestinal stromal tumors(GISTs).Methods The clinical presention,histological features,immunohistochemical characteristics were analyzed in 70 cases of GISTs.Results The clinical presention and imagine findings were similar with the gastrointestinal neoplasms,the pathological characteristics were similar with that in smooth muscle tumors and neurogenic tumors.100%(70/70) of cases highly expressed CD117,75.5%(53/70) of cases expressed CD34,40%(28/70) of cases expressed S-100.Desmin,Actin,MBP and CK were all negatively expressed in 70cases.Conclusion GISTs are not common tumors in gastrointestinal neoplasms.The clinic-pathological characteristics are similar with that in the smooth muscle tumors and neurogenic tumors.CD117 and CD34 postivity are the most valuable factor in diagnosing of GIST.The theropy and prognosis of GIST are different with that in the other gastroinal mesenchmal neoplasms.
