贫血患者网织红细胞及其荧光强度检测的临床意义

摘    要:目的探讨流式细胞术加核酸荧光染色技术在网织红细胞(Ret)及其荧光强度分析中的临床应用价值。方法用SysmexXT-2000i全自动血液分析仪分析不同类型贫血患者的网织红细胞(Ret)和荧光强度的变化。结果285例贫血患者与102例正常人群比较,其Ret绝对值及其Ret百分比有不同程度的升高、轻度降低或显著降低;荧光强度的变化差异较大,表现为低荧光强度网织红细胞比率(LFR)降低,幼稚网织红细胞比率(IRF)、中荧光强度网织红细胞比率(MFR)和高荧光强度网织红细胞比率(HFR)升高,差异均有统计学意义(P<0.05或P<0.01)。结论全自动血液分析仪对网织红细胞的自动化分析测量细胞多,避免了主观因素导致的误差,操作简便、快速、灵敏、重复性好、误差小、结果准确,方法易于标准化。网织红细胞及其荧光强度分析可作为某些贫血鉴别诊断的初筛指标及骨髓造血系统抑制和恢复较敏感的指标,对判断和监测骨髓的损伤程度和预后具有重要的临床意义。

Abstract:Objective To explore the clinical application value of the flow cytometry and fluorescein staining technique in the analysis of RET and its fluorescence intensity.Methods To analyze the variation of RET in the anaemia patients with different type and fluorescence intensity with Sysmex XT-2000i automatic blood analyzer.(P<0.05 orP<0.01).Results Compared 285 anaemia patients with 102 normal people,both of the modulus and percentage of RET increased to some different level,dropped slightly or obviously;LFR dropped,IRF,MFR and HFR increased significantly.Conclusion The automatic blood analyzer measures and analyzes so many cells that we can avoid the error induced by subjective factors and also it is fast,sensitive,repeatable,easy to mampulated and set standard with fewer error and accurate result.The analysis of RET and fluorescence intensity can be the preliminary screening target of differential diagnosis of some anaemia and the sensitive target of restraining and recovering of marrow hematopoietic system.It can be of great clinically significance in judging and monitoring the degree of marrow injury and its prognosis.
