老年糖尿病患者糖化血红蛋白水平对心脏结构和功能的影响

摘    要:目的探讨老年糖尿病患者不同糖化血红蛋白水平对心脏结构和功能的影响。方法以HbA1c7%作为界点将117例老年糖尿病患者分为A组(HbA1c≤7%)和B组(HbA1c〉7%),利用彩色超声诊断仪进行彩色多普勒超声心动图测量心脏结构指标IVSTd(室间隔舒张末期厚度)、LVPWTd(左室后壁舒张末期厚度)、LVDd(左室舒张末期内径)、LVDs(左室收缩末期内径),LVMW(左心室重量)及心功能指标E/A(舒张早期最大血流速度/舒张晚期最大血流速度)、EF(射血分数)、FS(短轴缩短分数)。结果B组与A组比较IVSTd、LVPWTdLVDd、LVDs,LVMW逐渐升高并有统计学意义(P〈0.05);左室收缩及舒张功能两组之间差异无统计学意义。结论老年糖尿病患者在HbA1c〉7%时对心脏结构有一定影响。

Abstract:Objective To study the effects of Glycated hemoglobin level of control on cardiac structure and function in elderly diabetes mellitus subjects. Methods 117 elderly diabetes mellitus patients were divided into A group (HbAl c ≤ 7% ) and B group ( HbAlc 〉7% ) by Glycated hemoglobin different levels, to evaluate their cardiac structure and function according as the resuhs of echo- cardiography measurements. Rusults Interventricular septum thickness of end - diastolr (IVSTd) , Ventricular posterior wall thickness of end -diastole (LVPWTd) , Left Ventricular end diastolic dimension (LVDd) , Left ventricular end systolic dimension (LVDs) and left Ventricular mass weight (LVMW) in group B were significantly differences compared with group A. Function of LV diastole and function of LV systole were not quite different. Conclusion It could affect the cardiac structure when HbAlc 〉7% in elderly diabetes mellitus subjects.
