三种血清肿瘤标志物联合检测在肺癌诊断中的应用研究

目的探讨癌胚抗原（CEA）、神经元特异性烯醇化酶（NSE）和鳞状细胞癌抗原（SCC）辅助诊断肺癌的临床应用。方法用化学发光免疫分析法测定133例肺癌和169例良性疾病患者血清CEA、NSE和SCC水平。评价它们对肺癌的诊断价值。结果单项CEA、NSE和SCC检测的诊断灵敏度、诊断特异性、阳性预测值、阴性预测值、阳性似然比和阴性似然比分别为49.6%、89.3%、78.6%、69.3%、4.8和0.56,41.4%、91.1%、78.6%、66.4%、4.7和0.64,39.8%、92.3%、80.3%、66.1%、5.2和0.65。二项联合平行试验的诊断灵敏度约60%,特异性大于85%。三项联合平行试验的诊断灵敏度为71.4%,特异性为85%。结论 CEA、NSE和SCC联合平行试验可明显提高对肺癌的诊断灵敏度,有利于肺癌的早期诊断。

Clinical application evaluation of three tumor markers in diagnosis of lung cancer

Objective To evaluate the clinical values of measuring carcinoembryonic antigen（CEA）,neuron specific enolase（NSE）,squamous cell carcinoma antigen（SCC）for diagnosis of lung cancer.Methods These tumor markers were measured in sera from 133 patients with lung cancer and 169 patients with benign lung diseases.Results The sensitivity,specificity,positive predictive value,negative predictive value,positive likelihood ratio and negative likelihood ratio of CEA,NSE and SCC were 49.6%、 89.3%、78.6%、69.3%、4.8 and 0.56、41.4%、91.1%、78.6%、66.4%、4.7 and 0.64、39.8%、92.3%、80.3%、66.1%、5.2 and 0.65,respectively.The sensitivity of two tumor markers combined measurement was about 60%,the specificity was more than 85%.The sensitivity and specificity of three tumor markers combined measurement were 71.4% and 85% respectively.Conclusion The combined measurement of serum CEA,NSE and SCC significantly increased the sensitivity for diagnosis of lung cancer and provide useful information for early diagnosis of disease in patients with lung cancer.
