乳腺癌根治术后并发皮下积液及皮瓣坏死74例分析

目的　探讨乳腺癌根治术后并发皮下积液和 (或 )皮瓣坏死的原因及预防处理方法。方法　对1996～ 2 0 0 1年乳腺癌术后并发皮下积液及皮瓣坏死的 74例病人进行了回顾性分析。结果　皮下积液多发生在腋窝 ,也见于皮瓣与胸壁之间 ,与术后引流不畅 ,局部包扎不当等有关。皮瓣坏死主要发生在电刀游离皮瓣处 ,相关因素包括皮瓣分离过薄 ,电刀功率过大 ,切口张力大等。结论　皮下积液与皮瓣坏死的发生因素主要与手术及术后处理不当有关 ,通过适当的预防措施可减少发生。

The analysis of 74 cases of postoperative complications of the accumulaive liquid of subcutanea and skin flap necrosis after permanent curing operation for breast cancer

Objective To explore the reason, preventing and curing principle of accumulative liquid of subcutanea and skin flap necrosis after permanent curing operation for breast cancer.Methods To conduct a retrospective analysis of the accumulative liquid of subcutanea and skin flap necrosis of the 74 patients from 1996 to 2001.Results Accumulative liquid of subcutanea most often appears in the armpit, and it also appears between skin flap and chest wall, which results from poor drainage after operation, local improper packing etc. Skin flap necrosis mainly lies in where the skin flap is cut with the electric knife, and it is mainly caused by the excessive thin separation of skin flap, excessive powerful electric knife, large extending force of the cut and etc .Conclusion Accumulative liquid of subcutanea and skin flap necrosis mainly result from improper treatment during operation or postoperatively, and it can be reduced by appropriate preventive measures.
