低位直肠癌保肛手术患者76例报告

目的评估吻合器骶前吻合术在低位直肠癌保肛术中应用的安全性和有效性。方法2002年1月～2009年10月为76例低位直肠癌患者，施行吻合器骶前吻合术，DukesA期32例，B期37例，C期7例。肿瘤距肛缘5cm者18例，6cm者32例，7cm者26例。结果直肠切除和肠吻合过程顺利，术出现吻合口裂开。吻合口瘘4例（5．3％），盆腔感染4例〈5．3％），吻合口狭窄16例（21．1％），盆腔局部复发2例〈（2．6％），水后3～6个月排便功能均控制良好，结论吻合器骶前吻合术安全可靠，可完成低值直肠癌的保肛手术。

A report of the sphincter preselwing surgei in low rectal carcinoma on 76 cases

Objective To investigate the salety and efficacy of presacrarn treating in low rectal carcinoma with staple. Methods From Jaouary7 2002 to October 2009,the above technique was performed on 76 eases patients including 32 cases of Dukes A ,37cases of Dukes B and 7eases of Dukes C. The distance between the edge of the sphincter and the lower margin of the minor was 5 cm（18cases） ,6 cm（32cases） and 7 cm（26cases）. Results Seventy - six patients were operated suceesstblly with the above techuigue. All patient were followed up complications included anastomotic leakage in 4 eases （5.3%） ,infectlon of pelvic cavity in 4 cases （5.3%）, anastomofic stenosis in 16cases （21.5%） and local reeurrence in 2 cases（2.6% ）. It may complete the preservation of anal sphincter of low rectal carcinoma. Defecation was under satisfactoly control postoperatively 3 - 6 months. Conclusion Presaeram treating with staple is secure and allow.
