终末期尿毒症并发神经精神症状临床分析

目的寻找终末期尿毒症并发神经精神症状的原因,提高终末期尿毒症的临床诊治水平。方法回顾性分析56例出现神经精神症状的终末期尿毒症患者临床资料,将其分为尿毒症脑病组(A)组,共30例;透析性脑病组(B)组,共26例。并观察两组辅助检查、治疗效果。结果终末期尿毒症出现神经精神症状常见原因为:(1)尿毒症性脑病主要是尿毒症毒素蓄积,代谢性酸中毒,电解质紊乱;(2)透析性脑病主要是失衡综合征,透析欠充分,脑血管病变,精神压力。结论终末期尿毒症出现神经精神症状与多因素有关。治疗上应积极治疗原发病、并发症,预防动脉硬化,选择恰当的透析方式、频率,并要注意排除脑血管器质性病变。

Clinical analysis of complicated neuropsychic symptom in end - stage uremia

Objective To study the reasons of complicated neuropsychic symptom in end-stage uremia,in order to improve the diagnosis and treatment effect of this disease.Methods Clinical materials of neuropsychic symptom in 56 patients with end-stage uremia were retrospectively analyzed.The patients were divided into 2 groups,and they were 30 patients with uremic encephalopathy in the group A and 26 patients with dialysis encephalopathy in the group,B and results of lab tests and therapy were investigated.Results IT showed that the reasons of complicated neuropsychic symptom in end -stage uremia were as follows:①Uremia toxin storage,metabolic acidosis and electrolyte disturbance were the causes of uremic encephalopathy,and ② Balance disturbance syndrome,non-sufficient dialysis ,cerebrovascular disorders and mental burden were the causes of dialysis encephalopathy.Conclusion Neuropsychic symptom in end-stage uremia was related to multiple factors.In order to be cured,timely therapy of the primary affection and complication,prevention of arteriosclerosis,dialysis with proper method and frequency should be conducted.And pay attention to exclude cerebrovascular organic disease.
