健康教育在社区慢性病管理中的应用

目的探讨社区健康教育对慢性病患者管理的效果。方法在大庆市为民社区筛查出患有高血压、糖尿病的患者892例，建立慢性病档案，对患者给予定期随访，针对性指导用药外加强健康教育，18个月后统计分析患者健康教育前后血压、血糖变化、生活方式及遵医行为变化。结果患者干预前后对比有显著性差异，健康知识和自我保健能力明显提高；血压、血糖水平下降，均有显著性差异（P〈0．01）。结论社区健康教育可以改善患者的生活方式和遵医行为，促使疾病达到满意控制效果。

The Application of Health Education in the Management of Community Chronic Disease

Objective To study the effect of community health education on management chronic disease patients. Methods All 892 residents with hypertension, diabetes of Daqing Weimin community were managed and taken community health education. Results Health knowledge and self-care were significantly improved before and after taking community health education （P 〈 0.01 ）. Patients life style and behavior interverltion as medical, blood pressure and blood sugar levels were significantly decreased （P 〈 0.01 ）. Conclusion Community health education can improve the patients lifestyle and compliance behavior, so patients treat and prevent disease from passively into actively, the disease controls prompting to achieve satisfactory effect.
