更昔洛韦治疗小儿巨细胞病毒性肝炎的疗效观察

目的探讨更昔洛韦治疗小儿巨细胞病毒性（CMV）肝炎的临床疗效。方法将2007年1月～2010年12月我院收治的46例巨细胞病毒性肝炎患儿根据人院先后顺序随机分为治疗组27例（应用更昔洛韦治疗）及对照组19例（应用病毒唑治疗），观察比较两组的疗效、治疗前后CMV病毒检测结果、患儿sB及ALT复常时间和ALT复升率。结果治疗组的总有效率（92．6％）明显高于对照组（63．2％），差异有统计学意义（P〈0．01）；治疗组CMV—IgM转阴例数、CMV—DNA下降例数及总阴转率与对照组比较，差异有统计学意义（P〈0．01）；治疗组sB、ALT复常时间明显短于对照组（P〈0．01）；治疗组ALT复升率明显低于对照组（P〈0．01）。结论更昔洛韦治疗小儿CMV肝炎疗效确切，安全、副作用小，值得临床推广。

The Efficacy of Ganciclovir in Treatment of Cytomegalovirus Hepatitis in Children

Objective To investigate the ganciclovir in treatment of cytomegalovirus （ CMV ） hepatitis in children. Methods From January 2007 to December 2010,46 cases of giant cell hepatitis in children admitted to our hospital were randomly divided into treatment group 27 cases in it, application of ganciclovir treatment for 4 weeks and the control group 19 cases in it, application of gancielovir treatment for 2 weeks. Two groups were observed and compared the efficacy of CMV virus test results before and after treatment, children with SB and time to ALT normalization, ALT recovery increase ratio. Results The overall response rate （ 92.6% ） was significantly higher （ 63.2% ）, the difference was statistically significant （ P 〈 0.01 ） .CMV-IgM negative in the treatment group the number of cases, CMV-DNA and total number of cases decreased conversion rate compared with the control group, the difference were statistically significant（ P 〈 0.01 ）. SB, ALT normalization were significantly shorter in treatment group than in the control group（ P 〈 0.01 ）. ALT recovery increase of treatment group was significantly lower than that of the control, group （ P 〈 0.01 ）. Conclusion Ganciclovir in treatment of children with CMV hepatitis efficacy, safety, side effects, worthy of promotion.
