胰岛素泵与多次皮下注射治疗妊娠期糖尿病的对比研究

目的观察胰岛素泵强化治疗妊娠期糖尿病（GDM）的疗效。方法选择76例GDM患者随机分为两组，胰岛素泵持续皮F输注胰岛素组为治疗组，多次皮下注射胰岛素组为对照组。比较两组血糖控制水平、达标时间、低血糖的发生及妊娠并发症的发生情况。结果两组血糖控制水平相比差异无统计学意义（P〉0．05），但治疗组血糖控制达标天数较对照组缩短，低血糖发生较对照组少，妊娠并发症如妊娠期高血压、羊水过多、早产的发生率均比对照组低，两组相比差异有统计学意义（P〈0．05）。结论胰岛素泵强化治疗妊娠期糖尿病能有效控制血糖，并可减少低血糖的发生，减少妊娠并发症，优于传统的胰岛素多次皮下注射方式。

Comparison of Treatment by Insulin Pump and Multiple Subcutaneous Insulin Injection in Treating Gestational Diabetes Mellitus

Objective To evaluate the effect of intensified treatment by insulin pump in gestational diabetes mellitus （ GDM ）. Methods All 76 GDM patients were divided into two groups randomly, patients in treatment group were treated with continuous subcutaneous insulin infusion （ CSII ）, patients in control group were treated with multiple subcutaneous insulin injection （ MSII ）, and to evaluat the blood glucose level, time for the successful treatment, hypoglycemia and maternal complications. Results The difference of decreasing blood glucose level in 2 groups was not significantly （ P 〉 0.05 ） .However, the CSII group reached the glucose target more quickly than the control group. The hypoglycemia frequency were significantly decreased in the CSII group versus MSII group.The incidence of pregnancy induced hypertension, polyhydramnios, preterm delivery were also significantly lower in CSII group compared with MSII group （ P 〈 0.05 ）. Conclusion Intensified insulin pump treatment on GDM can control hyperglycemia rapidly and decrease the incident of hypoglycemia and pregnancy complications.The effect of intensified pump treatment is better than traditional multiple subcutaneous insulin injection regimen.
