冠延长术在固定义齿修复中的临床应用

摘    要:目的探讨冠延长术后的基牙在固定义齿修复中的临床效果及相关问题。方法选取20例残冠残根达龈下和15例临床牙冠颌龈距过小且被选为固定义齿基牙的病例,行冠延长术,然后于6~8周后进行固定义齿修复。修复后3、6、12个月随访复诊,检查义齿咀嚼功能恢复情况、固位体边缘、龈沟深度、龈沟出血指数。结果固定义齿修复后3、6、12个月随访复诊,义齿咀嚼功能恢复理想,固位良好,边缘密合,牙龈无红肿。实验牙与对照牙的龈沟深度、龈沟出血指数差异均无统计学意义(P〈0.05)。结论对于残冠残根及临床牙冠颌龈距过短者作为基牙,冠延长术是提高固定修复效果的一种有效方法。

Application of Crown Lengthening Surgery in Fixed Partial Denture Restoration

Abstract:Objective To study the clinical results and related factors of crown lengthening surgery for abutment teeth in fixed partial denture treatment. Methods Twenty cases with residual crowns and roots no higher than gingivas and 15 cases with short clinic tooth crowns were selected as abutments of fixed partial dentures.These teeth were performed with crown lengthening surgery, 6 to 8 weeks after which the fixed dentures were manufactured. Follow-up examinations were respectively taken 3 months,6 months and 12 months after the treatment, including the examination of masticatory function, crowns margins, sulcular depth and sulcus bleeding index. Results Follow-up examinations were respectively taken 3 months,6 months and 12 months after the treatment. The dentures showed good masticatory function and strong retention force, margins were sealed well, and gingival swelling wasn' t found in clinic. There were no statistical significance ( P 〉 0.05 ) in sulcular depth and sulcus bleeding index between experimental teeth and control teeth . Conclusion Abutment teeth with short clinical crowns or residual crowns or residual roots, crown lengthening surgery is an effective method to increase retention of dentures.
