外固定架加压联合重建钢板治疗陈旧性Tile B1型骨盆骨折的效果观察

目的探讨外固定架阶段加压联合重建钢板治疗陈旧性TileB1型骨盆骨折的疗效。方法选择我院2009年8月-2010年12月收治的陈旧性TileB1型骨盆骨折患者15例。治疗时先对患者行骨盆外固定架固定；再分阶段逐渐加压，使耻骨联合分离处逐渐闭合最后行内固定手术治疗。结果对患者随访半年以上，切口均I期愈合。术后半年，患者耻骨联合间距与术前比较有显著差异（P〈0．05）；骨盆后环部的骨折和脱位均获骨性愈合。结论外固定架阶段加压联合重建钢板治疗陈旧性TileB1型骨盆骨折疗效显著，耻骨联合分离间距明显缩小，骨折或脱位均可获骨性愈合，值得临床推广应用。

Study on Treating Old Tile B1 Pelvic Fracture with Staged Pressurization Using External Fixator in Combination with Reconstruction Plate

Objective To discuss the effect of treating old Tile B1 pelvic fracture with staged pressurization using external fixator in combination with reconstruction plate. Methods Selected 15 old Tile B1 pelvic fracture patients treated in hospital from August 2009 to December 2010. Firstly, treated patients with external fixator; Secondly, pressurized by stages, made pubic symphysis separation gradually closed. Finally, performed internal fixation treatment. Results Patients were followed up more than half a year, incisions healed by first intention. Half a year after surgery, pubic symphysis distance showed significant difference （P 〈 0.05 ） compared to pre-operative situation; Fracture and dislocation of the posterior structure of pelvis acquired bony healing. Conclusion Evident effect of treating old Tile B1 pelvic fracture with staged pressurization using external fixator in combination with reconstruction plate was found, pubic symphysis distance dramatically decreased, both fracture and dislocation could acquire bony healing, clinical application are recommended.
