度洛西汀与氟西汀对首发抑郁症治疗的比较观察

目的比较观察度洛西汀与氟西汀治疗首发抑郁症的临床效果。方法2010年10月～2011年2月，76例首发抑郁症患者随机分为度洛西汀组和氟西汀组，每组38例，对比观察两组的临床疗效、HAMD、HAMA及不良反应。结果度洛西汀组与氟西汀组的总有效率比较，差异无统计学意义（Х^2=0．592，P〉0．05）。两组治疗后HAMD评分、HAMA评分均较治疗前下降（t=3．926，4．137，4．513，6．275，P〈0．05）。并且，度洛西汀组治疗后HAMD评分、HAMA评分均低于同期氟西汀组（t=3．488，3．951，P〈0．05）。两组不良反应发生率比较，差异无统计学意义（Х^2=1．317，P〉0．05）。结论度洛西汀与氟西汀对首发抑郁症治疗的疗效相当，不良反应少，安全性高，但度洛西汀较氟西汀能够较快降低HAMD评分、HAMA评分，是治疗抑郁症较为理想的药物。

The Comparison Observation of Duloxetine and Fluoxetine in Treatment with First Episode Depression

Objective To compare the clinical effect of duloxetine and fluoxetine in treatment with first episode depression. Methods From October 2010 to February 2011,76 cases of first episode depression patients were divided into duloxetine group and fluoxetine group, each group with 38 cases. The clinical effect, HAMD, HAMA scores and adverse effect of the two groups were compared. Results There was no significant difference in clinical effect between the two groups （ Х^2=0-592, P 〉 0.05 ）. Compared with pre- treatment, the scores of HAMD, HAMA were significantly decreased after treatment in both two groups （ t=3.926,4.137,4.513,6.275, P 〈 0.05 ）. Moreover, the scores of HAMD, HAMA in duloxetine group after treatment were significantly lower than those in fluoxetine group （t=3.488,3.951, P 〈 0.05 ）. There was no significant difference in adverse reaction rate between the two groups （ Х^2=1-317, P 〉 0.05 ）. Conclusion The duloxetine is equal to fluoxetine in clinical effect but can further low HAMD, HAMA scores, which is an idea drug for first episode depression.
