肝脏淋巴动力学异常改变的CT征象

回顾性分析了７００例肝硬化、肝硬化伴肝癌、肝转移瘤的ＣＴ扫描，９３例（１３．２８％）增强后有门静脉分支和／或肝内下腔静脉周围低密度区（ＰＶＬｓ），此征象反映了肝内淋巴动力学的异常改变，病理上的主要改变之一是两边淋巴管扩张，以上肝脏疾病中ＰＶＬｓ的出现主要原因是肝内淋巴液生成增多。讨论了肝内淋巴管扩张的解剖和病理生理基础。诊断要点和与肝内胆管扩张的鉴别方法。肝内淋巴动力学改变的影像诊断较困难，ＣＴ扫描显示的ＰＶＬｓ为无创性判断肝内淋巴动力学改变提供了安全简便有效的影像诊断方法。ｃＴ上ＰＶＬｓ作为反映肝内淋巴动力学改变的指标已被肯定，对其与各种疾病之间关系的深入研究，对于完善肝脏的ＣＴ诊断具有重要意义。

Abnormal Lymphatic Dynamics of Liver Demonstrated by CT

cases of abnormal lymphatic dynamics in different intrahepatic diseases were report-ed. The CT signs of abnormal intrahepatic lymphatic dynamics were the perivascular lucencies(PVLs),sur-rounding the portal vein and intrahepatic venacava. The PVLs are the dilated intrahepatic lymphatics. One ofthe causes for PVLs is increasing production of hepatic lymph as a compensatory mechanism to occur in thediseases. The basis of anatomy and pathology,criterion of diagnosis, differential diagnosis of PVLs and dilatedbibe ducts, and value of CT examination were discussed.It hadbeen confirmed that PVLs in CT screening re-flected intrahepatic lymph-dymamic. Further study is necessary to find out the relationship between PVLs and various diseases.
