人工髋关节置换术后深静脉血栓形成原因分析与防治

目的探讨人工髋关节置换术后并发深静血栓原因及防治措施。方法对我院1998年4月-2006年8月128例人工髋关节置换术后12例并发深静血栓的病例，对其病因及治疗进系统回顾性总结分析。结果12例并发深静脉血栓形成中主要原因有既往血栓史、心血管病、下肢静脉曲张或糖尿病6例，长期卧床或下肢长时间制动3例，手术时间过长及创伤过大3例。12例深静脉血栓采用肝素为主综合治疗措施治愈或缓解。结论既往血栓史、高血压心脑血管病、下肢静脉曲张及糖尿病等高危因素、患者卧床时间过长，手术创伤大、未能及时早期康复训练是深静脉血栓形成的主要原因。及时手术、康复训练、减少卧床及术中创伤，术前对高风险病人及术后并发深静脉血栓形成肝素的应用等是防治人工髋关节置换术后深静脉形成的关键。

Analysis of shape result and Prevention of deep venous thromboembolism after hip arthroplasty

Objective To explore the shape causes and therapeutic motheds of deep venous thromboembolism after hip arthroplasty. Method In our hospital,from April 1998 to August 2006, 12cases of 128cases with hip arthrosplasty had developed deep venoud thromboembolism,which result of disease and therapy are analysesed systemly. Result In this 12cases,the primary results are the thrombus, cardiopathy and vasal disease, and vasal dilation.6 cases had developed diabetes, 3 cases had beed laid and fixed at long time, 3 cases had beed overoperated and overtrauma. 12cases had been therapied by heprin and other motheds, and had gained batter or crue. Conclusion The primary causes of deep venous thromboemlolism are the thrombus, cardiopathy and vasal disease, vasal dilation, diabetes, and other danger factor; lay overlong,overtruama, and no train at time.The key of prevent it are to operate at time, to excise function, ,to decrease lay and truama, and treat with heparin to danger patient.
