急性脑血管疾病患者尿中N-乙酰-β-D-氨基葡萄糖苷同工酶的变化

摘    要:目的探讨尿液N-乙酰-β-D-氨基葡萄糖苷同工酶(以下简称NAG—B)与急性脑血管病的关系。方法采用对硝基酚比色法,分别测定了47例急性脑血管疾病患者(其中脑出血15例.脑梗死32例)的尿液NAG同工酶谱、尿NAG与尿肌酐(Cr)的比值、血清NAG活力。结果脑梗死患者的尿液NAG—B、尿NAG/Cr和血NAG活力分别为对照组的1.30,2.93,1,23倍,脑出血患者的三项指标分别为对照组的1.33.3.13,1.30倍。患者组结果均明显高于对照组(P〈20.01)。同时重症组患者尿NAG活性较轻症组明显升高(P〈0.05),恢复期患者尿NAG活性较惠性期有明显下降(P〈0.01)。结论脑血管病患者尿NAG及其同工酶活性升高的程度与病情的严重性呈一致关系.测定尿NAG—B对衡量脑血管疾病患者病情的轻重和估计疗效具有一定的意义。

Abstract:Objective To evaluate relationship between urine NAG isoenzyme and acute cerebrovascular disease (CVD). Methods The activity of urine NAG isoenzyme, NAG/Cr and blood NAG isoenzyme in 47 patients with acute CVD (15 patients with brain hemorrhage and 32 patients with brain infarction) were examined by p-nitrobenzene chronometry. Results The activity of urine NAG-B, NAG/Cr and blood NAG isoenzyme in patients with brain hemorrhage and brain infarction were 1.3,2.93,1.23 and 1.33, 3.13, 1.3 fold higher than that of normal control group respectively. There was significant difference between the patients and the normal control group (P<0.01).The activity of urine NAG-B in severe case group was higher than that of mild case group(P<0.05). Significant decrease of urine NAG isoenzyme activity was found in patients of convalescent period compared than that in patients of acute period (P<0.01). Conclusion A relationship is found between activity of urine and severity in patients with CVD. Assessment of urine NAG isoenzyme activity is useful for evaluating severity of the disease and predicting the response to its therapy.
