原发性心脏肿瘤的临床及超声诊断特点

摘    要:目的探讨原发性心脏肿瘤的临床特点及超声心动图在其诊断方面的价值。方法回顾性分析2002~2010年在该院就诊的共52例心脏肿瘤患者,其中原发性良性心脏肿瘤31例,原发性恶性心脏肿瘤8例,继发性心脏恶性肿瘤13例,分别对以上患者的性别、发病年龄、发病部位、临床表现、并发症及多普勒超声诊断等方面进行对比分析。结果 (1)心脏肿瘤良性者占79.49%,其中,单纯黏液瘤占93.55%,女性发病(32例)明显高于男性(20例),平均年龄(51.84±15.01岁)明显低于男性(60.28±11.49岁),差别有统计学意义(P0.05);(2)心脏良性肿瘤发于房间隔左房面者占80.65%,恶性肿瘤多发生于不典型部位;(3)超声诊断黏液瘤,误诊率及漏诊率分别为6.25%,0。结论超声心动图对原发性心脏肿瘤诊断敏感性高,不典型部位肿瘤有恶性可能,要引起重视。

Clinical characteristics and ultrasonic diagnosis of the primary cardiac tumor

Abstract:Objective To investigate the characteristics of primary cardiac tumors and the diagnostic value of echocardiography in primary cardiac tumors.Methods A total of 52 patients who were diagnosed with primary cardiac tumors in our hospital between 2002 and 2010 were reviewed in this study.These patients included benign primary cardiac tumors in 31 cases,primary malignant cardiac tumors in 8 cases,secondary cardiac malignancy in 13 cases.Comparative analysis was taken respectively over the gender,age,disease location,clinical manifestations,complications,and ultrasound diagnosis.Results Benign cardiac tumors accounted for 79.49%,in which myxoma accounting for 93.55%.Female(32 cases) was significantly higher than male(20 cases),whose average age(51.84±15.01 years old) was significantly lower than male(60.28±11.49 years old)(P0.05).Benign tumors occurring in the left atrial septal surface accounted for 80.65%,while malignant tumors occurred mainly in the atypical.Echocardiography as a method to diagnose myxoma,the rates of misdiagnosis and missed diagnosis were 6.25% and 0 respectively.Conclusion Echocardiography is highly sensitive to diagnosis of primary cardiac tumors.Primary cardiac tumors in non-typical site may be malignant tumor,to which we should pay more attention.
