2004～2007年间酵母菌在长钢总医院临床分离率及耐药分析

目的了解本地区酵母菌感染状况及其耐药性,以指导临床用药。方法应用手工鉴定法和科玛嘉显色培养基鉴定法对212株念珠菌进行分离鉴定,用ATB-FUNGUS真菌药敏试验条检测酵母菌对6种抗真菌药物的敏感率。结果2004～2007年间送检标本4443份,检出酵母菌212株。检出率前三名的标本为咽拭子6.4%,痰5.4%,粪便5.3%。检出率呈逐年增长趋势,由2004年的2.6%增至2007年的7.3%。其中白色念珠菌134株,占63.2%。热带念珠菌33株,占15.6%。212株念珠菌对5-氟胞嘧啶、制霉菌素、两性霉素B、酮康唑、咪康唑、益康唑的耐药率在0%～30%。结论酵母菌感染的部位以呼吸道、肠道为主,菌种仍以白色念珠菌为主,对6种抗真菌药物具有较高的敏感率。

The clinical isolating rate and drug resistance of yeast fungi in Changgang General Hospital from 2004 to 2007

Objective To investigate the infection status of yeast fungi and their drug resistance,and instruct the clinic to use antifungal agents rationally.Methods CHRO Mager monilial coloration culture medium identification and conventionally manual i- dentification method were applied to isolating and identifying 212 strains of yeast fungi;ATB FUNGUS susceptibility test strip was employed to determining the sensitivity of yeast fungi to 6 kind of antifungal agents.Results 212 strains of yeast fungi were detec- ted out of the delivered 444 3 samples from 2004 to 2007.The pharyngeal swab had the highest isolating rate (6.4%),followed by sputum (5.4%) and stool sample (5.3%).The detection rate was gradually rising year by year,from 2.6% in 2004 to 7.3% in 2007.Among the total,Candida albicans were 134 strains (63.2%),and Candida tropicalis were 33 strains (15.6%).The resist- ant rate of 212 strains of yeast fungi were 0%-30% to 5-flucytosine,mycostatin,amphotericin,ketoconazole,miconazole and econ- azole.Conclusion The location of yeast fungus infection focuses on the respiratory tract and intestinal tract,and Candida albicans was a common causative fungus.The sensitivity of yeast fungi was higher to 6 kind of antifungal agents.
