| 摘 要： | 目的：观察社区-医院一体化管理模式对老年慢性心力衰竭（CHF）患者及家属健康教育达标率的影响和对患者生活质量的改善情况。方法：将社区内普查出的老年CHF患者按居住区域分为干预组和对照组，比较两组患者经过两年不同的处理方法后各项观察指标的差异。结果：干预组患者经过2年的社区-医院一体化管理模式的干预，在服药依从性、患者及家属的健康教育达标率、生活质量等方面，均优于单纯药物治疗的对照组患者，统计学差异显著（P〈0．01），干预组再住院率显著低于对照组（27％vs．76％，P〈0．01）。结论：社区-医院一体化管理可以明显提高老年CHF患者的服药依从性、改善生活质量，提高患者及家属的健康教育达标率。

| Abstract: | AIM： To explore the effect of community and hospital seamless management on quality of life and family health education in elderly patients with chronic heart failure （CHF）. METHODS： Elderly patients with CHF in the community were divided into two groups according to residential areas： experi- mental group of community and hospital seamless management and control group of only drug treatment. Differences between groups were compared after 2 years. RESULTS ： Medication compliance, quality of life and family health education in experiment group were significantly better than those in the control group （P 〈0. 01 ）. CONCLUSION： Community and hospital seamless management markedly improves medication compliance, quality of life and family health education in elderly patients with CHF. |
