手术治疗胫骨Pilon骨折的临床研究

目的探讨胫骨Pilon骨折的手术治疗时机,手术方法。方法随访1999～2005年治疗的胫骨Pilon骨折40例。骨折类型:一型23例,二型9例,三型8例;30例合并腓骨骨折。胫骨最终均用钢板内固定,腓骨用1/3管状钢板内固定。结果40例骨折全部愈合,根据Mazur踝关节症状与功能系统评价标准,优31例,良7例,可2例。优良率95%。结论对胫骨Pilon骨折针对不同的受伤机制与骨折类型选择手术时机,最终坚强内固定治疗胫骨Pilon骨折,效果优良,可早期活动踝关节,不致功能丧失。

Clinical study of operative treatment of Pilon fractures of the distal tibia

Objective To evaluate the timing of operation and methods of surgery of tibia Pilon fractures. Methods Forty patients with tibia Pilon fractures operatively treated between 1999-2005yr. were followed- up. The types of fracture according to Ruedi-Allgower: there were 23 cases of type Ⅰ fracture, 9 cases of type Ⅱ fracture,8 cases of type Ⅲ fracture.The 30 cases unite of fibula fracture.The tibia were reducted and internal fixed with dissection plate.The fibula were internal fixed with plate.Results According to Mazur criterion, the results of treatment were evaluated as excellent in 31 cases,good 7 cases, fair in 2 cases. The excellent and good outcome rate was 95%.Conclusion To aim directly at the cause of injured and the type of fracture,tibia Pilon fractures should be treated with strong internal fixation.This technique is of effect good for Pilon fracture.It should be early mobilization for Ankle joint,don't to functional incapacitation.
