尸体标本腹腔神经丛:磁共振图像后处理分析

摘    要:目的探讨MRI图像后处理技术在尸体标本腹腔神经节显示中的价值。方法对18具成人尸体标本进行解剖,确定胰腺周围腹腔神经节及神经丛主支的位置和形态,并对其大小进行测量,用马根维显对其进行标记,还原尸体各脏器的位置。使用GE1.5TExciteMR成像仪,以腹腔神经节为中心对尸体进行MRI三维快速扰相梯度回波序列冠状位和轴位容积扫描,将扫描后的容积数据传至GEAW4.1工作站进行多平面容积重建(MPVR)。MPVR图像上所测结果与尸体解剖比较。结果18具成人尸体标本腹腔神经节和神经丛主支在尸体标本解剖和MPVR影像上均位于T12至L1的上缘之间;其形态在MPVR图像和尸体标本解剖基本一致,多为薄片型,结节型或长条型少见。左右侧腹腔神经节的上下径分别为(15.07±5.16)mm和(13.18±3.62)mm,左右径为(22.74±10.13)mm和(25.01±6.11)mm,前后径为(2.83±0.45)mm和(2.08±0.46)mm。在MPVR图像上左右侧腹腔神经节的上下径分别为(15.89±0.53)mm和(13.53±1.51)mm,左右径为(22.44±1.11)mm和(25.03±1.81)mm,前后径为(2.79±0.49)mm和(2.37±0.21)mm。上述径线在尸体标本解剖和MPVR图像上的测量值无显著差异(P>0.05)。结论MRI图像后处理技术,能准确、立体地显示胰腺周围腹腔神经节及腹腔神经丛主支的位置、形态、大小和走向,为临床腹腔神经丛的扫描奠定了基础,并具有指导意义。

Abstract:Objective To study the application of processing original image technique of MR imaging in the depiction of celiac plexus in cadavers. Methods Eighteen cadavers were dissected, moving peritoneal organs such as liver and stomach to expose the celiac ganglia. The location, morphology of celiac ganglia, and their relationship to abutting structures were noted. The celiac ganglia were isolated and marked with gadolinium chelate. The moved organs were relocated, the abdomen was closed, and three dimensional (3D) fast spoiled gradient recalled (FSPGR) MR imaging was performed. The raw data of 3D FSPGR were transferred to GE AW 4.1 workstation and performed original image processing by using multiplanar volume reconstruction (MPVR). The measurements of celiac ganglia derived from MPVR images were compared with those from dissection of cadavers. Results All of the celiac ganglia in the 18 cadavers were located between T12-L1, and their morphology was laminar shape in most. No significant difference can be seen in these observations by dissection compared with those by MR imaging. The dimensions of the left and right celiac ganglia by dissection were, respectively, (15.07±5.16) mm and (13.18±3.62) mm for the superior-inferior diameter, (22.74±10.13) mm and (25.01±6.11) mm for left-right diameter, and (2.83±0.45) mm and (2.08±0.46) mm for anterior-posterior diameter. While the dimensions of the left and right celiac ganglia derived from MPVR images were, respectively, (15.89±0.53) mm and (13.53±1.51) mm for the superior-inferior diameter, (22.44±1.11) mm and (25.03±1.81) mm for left-right diameter, (2.79±0.49) mm and (2.37±0.21) mm for anterior-posterior diameter. No significant difference can be seen in the dimensions derived from dissection and from MPVR images (P>0.05). Conclusion Using processing original image technique of MR imaging can depict the position, morphology and dimension of the celiac ganglia and main branches of celiac plexus around pancreas in cadavers precisely and stereoscopically. This may be a reference for in-vivo celiac plexus MR imaging.
