浸润型肺结核合并糖尿病507例临床分析

目的 通过合并糖尿病的肺结核的临床分析，探讨二病并存时的临床特点及糖尿病控制对结核化疗疗效的影响。方法 对507例住院的肺结核合并糖尿病的患者的临床资料进行总结分析。结果 合并糖尿病的肺结核患者比单纯肺结核有病情较重、干酪病灶多、空洞多、排菌病例多及复治病例多的临床特点；在糖尿病稳定，血糖控制在8．4mmol／L以下，其痰菌阴转率、病灶显吸率与单纯性肺结核的疗效相似。结论 糖尿病与肺结核互有影响，以糖尿病对肺结核影响较大，合并糖尿病的肺结核化疗的疗效及预后与糖尿病控制情况有很大关系。

Clinical analysis of 507 cases of pulmonary tuberculosis complicated with diabetes mellitus

Objective To investigate the clinical characteristics, therapeutic regimen and outcome of pulmonary tuberculosis complicated with DM (diabetes mellitus) and effect between pulmonary tuberculosis and diabetes mellitus. Methods The clinical features, diagnosis, treatment and outcome of 507 cases of pulmonary tuberculosis complicated with DM were analyzed. These patients were treated in 309th hospital from January 1998 to December 2004. Results Compared with those without DM, the patients complicated with DM were more severe, and more caseous foci, more pulmonary cavities, more bacilli in sputum and more recurreneces. If diabetes mellitus was stable and blood glucose was less than 8.4 mmol/L, the outcome was similar to those without DM in respect of the sputum bacillus disappearance rate and the tuberculosis foci absorption. Conclusion The complication of diabetes mellitus may impeded the recovery of tuberculosis, so the control of blood glucose is important to the treatment and prognosis of pulmonary tuberculosis.
