超声显像在胃溃疡诊断中的应用价值

目的 研究分析超声显像在胃溃疡诊断中的应用价值。方法 49例经胃镜检查为胃溃疡的患，采用超声多切面、多角度观察胃部结构及邻近脏器，并与胃镜检查结果比较，统计得出超声检出符合率。结果 49例患中，超声诊断胃溃疡39例，总体检出符合率为79．6％；漏诊10例，占20．4％。39例中，胃角部溃疡27例，占69．2％；胃小弯部溃疡8例，占20．5％；胃大弯部溃疡4例，占10．3％。4例溃疡型胃癌全部检出。结论 在胃充盈良好的情况下，超声可清楚分辨胃壁的各层结构及其异常改变，根据面积大小及凹陷程度、胃壁增厚与胃蠕动等指标可鉴别溃疡的程度及性质，对于溃疡型胃癌的发生及其对周围脏器的影响也有很好的分辨率。因此，超声对胃部溃疡的诊断虽具有较高价值，但对较小浅表的溃疡仍可能漏诊。

Applicable Value of Ultrasonography in Gastric Ulcer Diagnosis

Objective To study and analyse the applicable value of ultrasonography in gastric ulcer diagnosis.Methods A total of 49 cases with gastric ulcer,who were diagnosed by gastroscopy,were scanned with ultrasound on stomachs and neighbour organs from multi-sections and multi-directions.The diagnostic results were compared with those of gastroscopy to calculate the coincidence rate.Results Thirty-nine of 49 cases were confirmed by ultrasonography and the overall coincidence rate was 79.6%.Ten cases(20.4%) were misdiagnosed.Among the 39 cases,27 cases (69.2%) were incisura angularis ulcer,8 cases(20.5%) were gastric body ulcer and 4 cases(10.3%) were gastric antrum ulcer.Conclusions 2-D ultrasonography is able to demonstrate the clear structures of the stomach wall stratifications and the morbid changes (e.g.,the ulcer area and location),and the therapeutic progress of gastric ulcer.2-D ultrasonography also has a good vision for gastric tumoral genesis and tumoral metastasis to the neighbour organs,especially to the individuals who were not suitable to undergo X-ray and gastroscopy examination,ultrasonography is the prior choice,but some superficial ulcers may be missed.After all,ultrasonography has good value in gastric ulcer diagnosis.
