Abstract: | The author presents a review of literature focusing of state-of-art of surgical management of epilepsy. The second part of review discusses basic principles of selection of patients with localization-caused epilepsy for surgical treatment, preoperative examination, substantiation of resection procedures, results of treatment. Special attention is given to controversial aspects of tactics of surgical management: neurophysiological and neuroimaging signs of epileptogenic zones, determination of required resection volume, value of preoperative invasive examination and intraoperative electrocorticography. Surgical complications, especially concerning medial temporal procedures, are also described. |
