尺骨冠突解剖学研究及其临床意义

目的:探讨国人尺骨冠突参数的特点,为临床修复和重建冠突提供参考依据。方法:对30具60侧新鲜成人尸体上肢作研究,分别测量男女尺骨冠突的高度,滑车切迹开口纵径,开角的大小以及冠突高1/2处在冠状面上的宽度,矢状面上的厚度等,并根据性别对上述参数进行比较,同时探讨肘关节尺侧副韧带前束在冠突内侧止点的位置与冠突高1/2处的关系。结果:正常成人男性尺骨冠突平均高度、宽度、厚度及滑车切迹开口纵径分别为18.43±0.56mm、22.53±0.78mm、8.17±0.32mm和20.93±0.34mm,女性为16.35±0.41mm、19.76±0.82mm、6.32±0.83mm和18.48±0.59mm,经统计学分析男性上述指标明显大于女性(P<0.05),与男性平均身高及尺骨长度比女性大有关,而开角的大小约30°,无性别差异(P>0.05),与身高及尺骨长度无关。根据肘关节尺侧副韧带前束在冠突内侧止点位置与冠突高1/2的关系,有3种类型。结论:根据正常成人尺骨冠突测量数据及肘关节尺侧前束在冠突内侧止点处的位置,可为临床重建尺骨冠突及尺侧副韧带前束提供参考依据。

Anatomy study of ulna coronoid process and its clinical significance

Objective:To explore the characteristics of ulna coronoid process in adult Chinese and to provide reference data for clinical repairing and reconstructing.Methods: Through thirty cadaveric upper extremities(sixty arms),the height,width, thickness, diameter and opening angle of ulna coronoid process were measured,and to compare these data between male and female. Furthermore, to explore the relations of the insertioin of the anterior bundle of the ulnar collateral ligaments(UCL) and half height of coronoid process.Results:The height,width,thickness,diameter of ulna coronoid process of the adlut male were 18.43±0.56mm,22.53±0.78mm,8.17±0.32mm and 20.93±0.34mm,of the female, were 16.35±0.41mm,±19.76±0.82mm,6.32±0.83mm and 18.48±0.59mm ,there was statistical significance between male and female(P<0.05),but the opening angle has no statistical significance(P>0.05)about 30 degrees. The insertion of the anterior bundle of UCL in coronoid process have three types.Conclusion: There data may serve asthe basis of clincal repairing and reconstructing coronoid process , according to the characteristics of ulna coronoid process in adult Chinese.
