甲状腺次全切除治疗甲状腺腺瘤的疗效观察

目的观察甲状腺次全切除术对甲状腺腺瘤的治疗效果,为治疗甲状腺腺瘤及预防术后并发症提供依据。方法对1993-2008年63例甲状腺腺瘤病人行甲状腺次全切除术,采用局部麻醉,术中采取不解剖暴露出喉返神经,在可能钳夹或结扎喉返神经操作时及时与患者对话,切除腺体时保留腺体背面部份的完整性,并安放有效引流等措施,术后跟踪随访,观察其复发和并发症情况。结果出院后进行门诊或电话随访,失去联系5例,随访58例,随访率92.1%,随访时间最长17年,最短2年,无1例复发,无喉返神经损伤,无甲状腺或甲状旁腺功能低下等手术并发症发生,取得较好效果。结论以熟悉甲状腺及其神经血管、周围组织的解剖及变异、细致精确的手术操作为前提,施行甲状腺次全切除治疗甲状腺瘤可以避免复发和并发症的发生,是治疗甲状腺腺瘤瘤可行、有效的治疗措施。
