川崎病患儿T细胞亚群表达及其临床意义

摘    要:目的探讨川崎病患儿血CD3,CD4,CD8,CD4/CD8的差异以及在预测冠状动脉病变风险中的临床意义。方法选取2004-01~2007-04在太原市妇幼保健院住院的川崎病患儿42例作为观察组,抽取静脉血,同时以40例正常体检儿童作为正常对照组抽取外周静脉血。用流式细胞仪检测CD3,CD4,CD8,CD4/CD8。用二维超声心动图观察观察组心脏冠状动脉病变。结果观察组患儿CD3,CD8较对照组显著降低(P<0.01),CD4,CD4/CD8显著升高(P<0.01)。冠状动脉病变组CD3,CD8明显低于非冠状动脉病变组(P<0.01),而CD4/CD8显著高于非冠状动脉病变组(P<0.01)。结论T细胞亚群失衡在川崎病的发病中起重要作用,对预测冠状病变发生有一定的意义。

Abstract:Objective To investigate the difference of CD3, CD4, CD8, CD4/CD8 in patients with Kawasaki disease(KD)and to explore the clinical value in predicting risks of coronary artery lesion(CAL). Methods Forty-two patients with KD were recruited as KD group in Taiyuan Hospital and Health Center for Woman and Children from January 2004 to April 2007, and forty normal children were chosen as control group. Peripheral venous blood samples were collected, and serum levels of CD3, CD4, CD8, CD4/CD8 were detected by flow cytometry. CAL was observed with 2D-echocardiography and coronary angiographer. Results Levels of CD3, CD8 were significantly lower in KD group than that in control group (P<0.01), but levels of CD4, CD4/CD8 were higher in KD group than in control group(P<0.01). Levels of CD3, CD8 were significantly lower in CAL than in NCAL (P<0.01),but level of CD4/CD8 was significantly higher in CAL than in NCAL (P<0.01). Conclusion The disorder of T subsets plays an important role in occurrence of KD and in predicting risks of coronary artery lesions.
