江苏省结核病定点医院模式的患者管理质量分析

目的比较江苏省结核病定点医院模式和疾控中心模式的患者管理质量差异,分析存在问题,为改善管理质量提供依据。方法以2010年12月为界限,将全省106个县(市、区)按结核病防治模式分为定点医院模式和疾控中心模式,通过结核病信息管理系统收集两种模式下2011年全省登记的全部结核病患者管理信息,比较两种模式下的管理差异。结果定点医院模式下的肺结核患者系统管理率(99.4%)略高于疾控中心模式(99.1%);定点医院模式下的肺结核患者治疗成功率(94.0%)、信息录入及时率(98.0%)、完整率(98.9%)均低于疾控中心模式(分别为96.0%、99.54%、99.3%,P均<0.001),但均显著高于国家规划要求。结论定点医院模式下肺结核患者管理的各项指标均达到国家规划要求,能适应不断发展的结核病防治工作需求,符合我国医疗机构改革方向,可因地制宜地稳步推行。

Analysis on tuberculosis patient management quality under designated hospital mode in Jiangsu Province

Objective To compare the tuberculosis （TB） patient management quality between designated hospital mode and Centers for Disease Control and Prevention（CDC） mode. To analyze problems and provide scientific basis to improve the pa- tient management quality. Methods Till December 2010, all 106 counties in Jiangsu Province were categorized into two modes： designated hospital mode and CDC mode. Management information of all registered TB patients was collected and com- pared {or difference of patients＇ management under 2 modes in 2011. Results The system management rate under the designat- ed hospital mode （99.4%） was higher than that under CDC mode （99.1%） ; under designated hospital mode, treatment suc- cess rate（94.0 %0 ）, timely rate and completeness rate of information input （98.0 % and 98.9 % respectively）, were lower than those under CDC mode （96.0% ,99.54% and 99.3% ,respectively. P%0. 001）. However, all rates of above indicators were higher than the national requirements. Conclusion Designated hospital mode of TB patients management satisfies national re- quirements and is capable to be adapted to the evolving needs of TB control and prevention tasks. Also, it is in line with reform of medical institutions and can be implemented locally step by step.
