切开复位内固定以及手法复位石膏外固定法在治疗老年骨质疏松性桡骨远端骨折效果及其对骨质疏松的影响评价

目的：探讨切开复位内固定以及手法复位石膏外固定两种固定方法在治疗老年骨质疏松性桡骨远端骨折临床效果及其对骨质疏松的影响。方法：回顾性分析入住我院的100例经临床诊断为老年骨质疏松性桡骨远端骨折患者的临床资料，随机分为两组，每组50例，分别给予手法复位石膏外固定术与切开复位内固定术，比较两组临床治疗效果、Mcbridge评分、QLQC30生活质量评分以及并发症发生率、骨密度。结果：根据经改良的Mcbridge腕关节评分标准，切开复位内固定组骨质功能恢复情况较手法复位石膏外固定法要好，且对骨密度的影响程度要小；根据QLQC-30生活质量评价标准，切开复位内固定组术后生存质量要明显优于手法复位石膏外固定组，差异具有统计学意义（P〈0．05）；切开复位内固定组优良率优于手法复位石膏外固定组，差异具有统计学意义（P〈0．05）；手法复位石膏外固定组骨密度下降速度要明显快于切开复位内固定组。结论：切开复位内固定治疗老年骨质疏松性桡骨远端骨折的临床效果要优于手法复位石膏外固定法，且切开复位内固定组患者术后生存质量优于手法复位石膏外固定组，应在临床治疗老年骨质疏松性桡骨远端骨折中加以推广并应用。

Effect comparison of open reduction plus internal fixation method and manual reduction plus plaster external fixation on senile osteoporotic distal radius fracture and their effects on oste- oporosis

Objective. To compare effects of open reduction plus internal fixation method and man ual reduction plus plaster external fixation on senile osteoporotic distal radius fracture and their effects on osteoporosis. Methods. Data of 100 senile patients clinically diagnosed as osteoporotic distal radius frae tures were retrospectively analyzed. Cases were randomly divided into two groups, with 50 cases in each, one group were treated with manual reduction plus plaster external fixation, while the other were treated with open reduction plus internal fixation. Clinical curative effects, QLQC-36 life quality score, Mcbridge score, bone density, as well as eompliccations were compared between the two groups. Results： The roodified Mcbridge wrist evaluation results showed open reduction plus internal fixation has smaller influence on bone density, higher excellent and good recovery rates but better function recovery effects than the mannal plaster external fixation method （P〈0.05）, life quality of the open reduction plus internal fixation group was also superior to that of the other group （P%0.05） according to the QLQC - 30 li~e quality eval- uation. Manual reduction plaster external fixation group treated group showed quicker seep of bone miner- al density decrease. Oonclusion： Comparing with manual reduction plus piaster external fixation method, open reduction plus internal fixation shows better effects on distal radius fracture in senile patients with os- teoporosis and the patients have better postoperative life quality, thus it＇s worthy of application in large scale.
