腹腔镜手术对重症急性胰腺炎患者RAAS及细胞因子的影响

目的：研究观察腹腔镜手术对重症急性胰腺炎（SAP）患者肾素～血管紧张素-醛固酮系统（RAAS）及细胞因子的影响。方法：选取行传统开腹手术治疗的43例SAP患者为对照组，同期采用腹腔镜手术治疗的43例患者设为观察组，然后将两组术前与术后1、3、5d的血清RAAS系统指标、炎性系统指标及其他疾病相关细胞因子水平进行比较。结果：观察组术后1、3、5d的RAAS系统指标、炎性系统指标及细胞间黏附分子-1（ICAM-1）均低于对照组，差异均具有统计学意义（P〈0．05）；两组其他疾病相关指标比较，差异无统计学意义（P〉0．05）。结论：腹腔镜手术对重症急性胰腺炎患者RAAS及炎性系统指标的影响更为积极，且对疾病状态的改善效果也较佳。

Influence of laparoscopic operation on RAAS and cytokines in patients with severe acute pancreatitis

Objective. To study and observe the influence of laparoscopic operation on RAAS and cytokines of patients with severe acute pancreatitis （SAP）. Methods. A total of 43 patients with severe a- cute pancreatitis scheduled for traditional open operation in our hospital were selected as control group,and 43 patients who scheduled for laparoscopic operation at the same time were selected as observation group, then the RAAS system indexes, inflammatory system indexes and other disease related indexes of two groups before the operation and at lst,3rd and 5th day after the operation were compared. Results. The se- rum RAAS system indexes,inflammatory system indexes and ICAM-1 of observation group at 1 st, 3rd and 5th day after the operation were all obviously lower than those of control group （P〉0.05）, while other disease related indexes of two groups were not significant different （P〉0.05）. Conclusion：The influence of laparoscopic operation for the RAAS and inflammatory system indexes of patients with severe acute panereatitis is more positive,and it improve symptoms of the disease too.
