BAFF与Bcl-2mRNA水平对多发性骨髓瘤发生、发展及预后的影响

摘    要:目的:探讨B细胞活化因子(BAFF)及B细胞淋巴瘤-2基因(Bcl-2)mRNA表达水平对多发性骨髓瘤临床诊断和预后评估的应用价值。方法:选取本院2011年4月~2013年1月收治的多发性骨髓瘤患者37例,以11例同期健康志愿者为对照组,以密度梯度离心法分离各组骨髓的单个核细胞,采用RT—PCR技术测定BAFF及Bcl-2的mRNA表达水平;同时,采取各组的空腹静脉血,以ELISA试剂盒测定肿瘤负荷标记物β2-微球蛋白(β2-MG)的水平,分析BAFF及Bcl-2的mRNA表达与血清β2-MG含量的相关性。结果:多发性骨髓瘤患者的BAFF及Bcl-2mRNA表达水平显著高于对照组(P〈0.05);同时,各分期患者之间亦有显著差异,随着分期的递增,BAFF及Bcl-2的mRNA表达水平依次升高(P〈0.05)。直线回归分析结果显示,BAFFmRNA表达水平与血清J32-MG含量呈直线正相关(R^2-7587,P〈0.05);Bcl-2的mRNA表达水平与血清β2-MG含量亦呈直线正相关(R^2-837,P〈0.05)。结论:BAFF及Bcl-2的mRNA表达水平不仅与多发性骨髓瘤的发病和进程密切相关,且能够较为准确地反映患者机体的肿瘤负荷程度,对多发性骨髓瘤的临床诊断和预后评估均有重要预测价值。

Abstract:Objective: To explore the application value of BAFF and Bcl-2 mRNA expression in clinical diagnosis and prognosis assessment of multiple myeloma. Methods: A total of 37 cases with multi- ple myeloma admitted in our hospital from April 2011 to January 2013 were selected and divided into I pe- riod group with 7 cases, II period group with 19 cases, andIlI period group with 11 cases by D-S stage; while, 11 healthy volunteer were selected as control group. Mononuclear cells of each bone marrow were extracted by density gradient centrifugation to determinate BAFF and Bcl-2 mRNA expression was meas- ured by using RT-PCR technique at the same time, fasting venous blood were extracted to determinate serous content of β2-MG by ELISA kits, which had relevance with BAFF and Bcl-2 mRNA expression were analyzed with statistical approach. Results: There was significant difference between multiple myelo- ma patients and volunteers in BAFF and Bcl-2 mRNA expression (P〈0.05) ; at the same time, there were also significant differences between stages of patients with a rising trend with stage increasing (P〈0.05). It showed by the linear regression analysis that BAFF mRNA expression and serous β2-MG content had positive linear correlation (R2 =7 587, P〈0.05); further more, Bcl-2 mRNA expression and serous β2- MG content also had positive linear correlation (R2 = 837, P〈0. 05). Oonclusion: We conclude that BAFF and Bcl-2 mRNA expression have important predictive value for the clinical diagnosis and prognosis of multiple myeloma which are no only closely related with the onset and processes of multiple myeloma but also able to reflect the extent of tumor load accurately.
