摘    要:目的:研究肝硬化患者血清胆汁酸的变化情况,探讨血清胆汁酸对肝硬化病情程度的评估作用。方法:以59例健康体检人员为对照,比较89例不同Child—pugh分级肝硬化患者各肝功能指标情况,研究各肝功能指标Child—pugh评分的相关性。结果:肝硬化组与对照组血清总胆汁酸(TBA)、总胆红素(TBIL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)水平差异均有统计学意义(P〈0.05),肝硬化组各肝功能指标均出现不同程度升高;不同Child—pugh分级肝硬化组任意两组TBA、TBIL水平比较差异均有统计学意义(P〈0.05),B级组ALT水平与c级组比较差异无统计学意义(P〉0.05),A级组AST水平与B级组比较差异无统计学意义(P〉0.05);TBA、TBIL与Child—pugh评分均呈正相关(r=0.692、0.529,P〈0.05)。结论:血清TBA与Child—pugh评分有较好的相关性,能准确反映肝硬化患者的病情严重程度,对肝硬化的早期诊断、病情评估均有重要的临床应用价值。

Abstract:Objective: To study the changes of serum bile acid in patients with liver cirrhosis, and to explore the role of serum bile acid in evaluation of liver cirrhosis severity. Methods: A total of 59 healthy volunteers served as control group, 89 patients with cirrhosis at different Child-pugh classification were selected as cirrhosis group. Correlation between indexes of liver function and Child-pugh score was analyzed. Results. There were significant differences in TBA, TBIL, ALT and AST between two groups (P〈0.05) ,and all indexes in cirrhosis group were increased at different degree; The differences in TBA, TBIL were significant among groups at different Child-pugh classification (P〈0.05). The difference in ALT was not significant between B and C grade level significant between group A and B grade level group group (P〈 0.05), and the difference in AST was not (P〉0.05)〈 TBA (r=0. 692, P〈0.05), TBIL (r=0. 529,P〈0.05) and Child-pugh score was positively correlated.
