喉鳞癌患者凝血功能状态及其临床意义

目的：探讨喉鳞癌患者凝血状态的改变及其与肿瘤临床分期、转移的关系。方法：选取113例喉鳞癌患者（试验组）与65例喉良性疾病患者（对照组）为研究对象，分析血浆凝血酶原时间（胛）、活化部分凝血活酶时间（hgrr）、凝血酶时间（TT）、纤维蛋白原（Fib）在两组问的差异及其与喉鳞癌临床分期、转移的关系。结果：喉鳞癌患者血浆APTT比对照组显著缩短（P〈0．05），其中转移组比未转移组显著缩短（P〈0．05）；喉鳞癌患者血浆Fib高于对照组（P〈0．05），其中转移组明显高于未转移组（P〈0．05），Ⅲ＋Ⅳ期组明显高于I＋Ⅱ期组（P〈0．05）；PT及TT在各组间无显著差异（P〉0．05）。结论：喉鳞癌患者血液呈高凝状态，检测血浆APTT及Fib有助于喉鳞癌患者的临床诊治和预后评估。

Blood coagulation and its clinical significance in patients with laryngeal squamous cell carcinoma

Objective：To explore the variation of the coagulation indexes in patients with laryngeal squamous cell carcinoma（LSCC） and the relationship between the coagulable abnormality and advanced laryngeal squamous cell carcinoma. Methods：The indexes of prothrombin time （PT）, activated partial thromboplastin time （APTT）, thrombin time （TT） and fibrinogen （Fib） were detected by using automatic blood coagulation testing machine in 65 patients with laryngeal benign disease（ control group ） and 113 patients with LSCC （test group ）. The data were analysed by SPSS 11.0. Results：The index of APTT in test group was shorter than that in control group（ P 〈 0.05 ）. In patients with LSCC ,there was a significant difference between the primary tumor group and the group with the neck metastasis of lymph node （ P 〈 0.05 ）. The index of Fib in test group was longer than that in control group（ P 〈 0.05 ）, and it in- creased with the clinical stage（ P 〈 0.05 ）. The fibrinogen level was higher in patients with lymph node metastasis than those without（ P 〈 0.05 ）. However, PT and ＇IT were not statistically different among groups （P 〉 0.05 ）. Conclusion： The patients with LSCC are in the hypercoagulable state. Fib and APTT may be the useful indexes for diagnosis, thera- py and prognosis in patients with LSCC.
