4D PRO悬吊训练联合Mulligan技术治疗慢性非特异性腰痛患者的临床观察及表面肌电研究*

摘    要:目的:探讨4D PRO悬吊训练联合Mulligan技术在慢性非特异性腰痛患者中的治疗效果及表面肌电影响。方法:选择2021年4月~2023年1月慢性非特异性腰痛患者120例为对象,随机数字表法分为两组,各60例。对照组采用常规保守治疗,联合组在对照组基础采用4D PRO悬吊训练联合Mulligan技术治疗,两组均完成4周治疗,疗程完毕后进行6个月的随访,比较两组视觉模拟疼痛(VAS)评分、腰部功能ODI评分、表面肌电水平、临床疗效及复发率。结果:联合组治疗4周后VAS评分、腰部功能ODI评分均低于对照组(P<0.05);联合组治疗4周后多裂肌和腹横肌的表面肌电RMS、MF、MPF水平均低于对照组(P<0.05);联合组4周疗效率高于对照组(P<0.05),且疗效后6个月复发率低于对照组(P<0.05)。结论:4D PRO悬吊训练联合Mulligan技术用于慢性非特异性腰痛患者中可获得较高的近期疗效,改善患者腰部功能,减轻患者疼痛,降低远期复发率,值得推广应用。

Abstract:Objective: To investigate the therapeutic effect of 4DPRO suspension training combined with Mulligan technique in patients with chronic nonspecific low back pain and the influence of surface electromyography. Methods: 120 patients with chronic nonspecific low back pain from April 2021 to January 2023 were randomly divided into two groups, 60 cases in each group. The control group was treated with conventional conservative treatment, while the combination group was treated with portable 4dpro suspension training combined with Mulligan technology on the basis of the control group. Both groups completed 4 weeks of treatment, and were followed up for 6 months after the course of treatment. The visual analogue pain (VAS) score, ODI score, surface electromyography level, clinical efficacy and recurrence rate of the two groups were compared. Results: The VAS score and ODI score of lumbar function in the combination group were lower than those in the control group at 4 weeks after treatment (P< 0.05); The surface myoelectric RMS MF MPF levels of multifidus muscle and transverse abdominalis muscle in combination group were lower than those of control group (P<0.05); The 4-week efficacy rate of the combined group was higher than that of the control group(P<0.05), and the recurrence rate at 6 months after treatment was lower than that of control group (P<0.05). Conclusion: 4DPRO suspension training combined with Mulligan technology can obtain high short-term efficacy in patients with chronic non-specific low back pain, improve patients'' waist function, reduce patients'' pain, and reduce the long-term recurrence rate, which is worthy of promotion and application.
