腰椎间盘术后椎间盘炎的诊断与治疗方法探讨

目的探讨腰椎间盘术后椎间盘炎的临床特征，诊断与治疗方法。方法自1994年7月-2004年7月共收治9例腰椎间盘术后椎间盘炎患者，平均年龄48岁，近期内均接受过腰椎间盘手术，其中2例使用椎间盘手术。全组患者血沉均升高（40-110mm/h)，均经MRI检查确诊，6例症状典型，疼痛较重的患者行前路病灶清除植骨融合术，3例症状不典型，疼痛较轻患者予非手术治疗。结果 行前路病灶清除植骨融合术患者，疗效较好，症状缓解明显，保守治疗病程较长，病精常有反复，但一般预后良好。结论结合患者的临床特性，经MRI检查可以作出诊断。根据患者病情的具体情况，选择手术治疗或非手术治疗。

The diagnosis and treatment strategy of discitis after lumbar disc surgery

Objective To evaluate the clinical characteristics, diagnosis and treatment strategy of discitis after lumbar disc surgery.Methods There were 9 cases of lumbar discitis from July 1994 to July 2004 with an average age of 48 years. They all got a lumbar disc surgery not long before they got the discitis, two of whom had a microendoscopic discectomy. All the patients had erythrocyte sedimentation rates rise (range 40-110mm/h), and took MRI examination to be diagnosed. Six cases had typical symptoms, and the patients who had serious symptoms underwent anterior debridement and interbody fusion. The symptoms of the other 3 cases were not typical, and the patients who had negligible symptoms got conservative treatment. Results The patients who received surgical intervention had a good curative effect and the patients who received conservative treatment had a long course and a recurrence condition but also were healed. Conclusion Combined with the clinical characteristics, diagnosis can be affirmed by MRI scans. According to the patient's condition, surgical intervention or conservative treatment can be chosen alternatively as a treatment strategy for lumbar discitis.
