密闭式吸痰管不同更换时间在机械通气患者中应用价值的比较

摘    要:目的对比密闭式吸痰管不同更换时间在机械通气患者中的应用价值。方法选择使用密闭式吸痰管吸痰的机械通气患者80例,随机分为A组26例、B组27例、C组27例,每次密闭式吸痰管更换时间分别为24 h、72 h、120 h,对比三组呼吸机相关性肺炎发生率、吸痰管前端培养的菌落计数、定植率。结果 A、B、C三组患者的呼吸机相关性肺炎发生率分别为11.54%、7.41%、3.70%,差异无统计学意义(P>0.05);三组患者的菌落计数分别为(6100±610)cfu/mL、(9 920±1 440)cfu/mL、(273 000±1 670)cfu/mL,定植率分别为57.69%、70.37%、100.00%,差异均具有统计学意义(P<0.05)。结论密闭式吸痰管的不同更换时间对机械通气患者费用与安全具有差异性,每72小时更换一次的应用价值最高,值得临床推广与研究。

Abstract:Objective To compare the application value of different replacement time for closed suction tube in mechanical ventilation. Methods A total of 80 patients who had mechanical ventilation and closed suction tube for aspiration of sputum were randomly divided into 3 groups: Group A of 26 cases with replacement time of 24 hours, Group B of 27 cases with replacement time of 72 hours and Group C of 27 cases with replacement time of 120 hours, then the colony counts, the rates of bacterial colonization cultured in closed sputum aspirator tip and incidence rate of VAP were compared between three groups. Results The incidence rate of VAP in three groups was respectively 11.54%, 7.41% and 3.70%, there were no statistically significant differences(P〉0.05). The colony counts in three groups was re- spectively (6 100±610) cfu/mL, (9 920±1 440) cfu/mL, (273 000±1 670) cfu/mL, and the rate of bacterial colonization cultured in closed sputum aspirator tip in three groups was respectively 57.69%, 70.37% and 100.00%, which had statistically significant difference (/9〈0.05). Conclusion Different replacement time for closed suction tube in mechanical ventilation, has different cost and safety, the replacement time of 72 hours has the highest value, it is worthy of clinical popularization and research.
