部分性脾动脉栓塞联合自体骨髓干细胞移植治疗肝硬化脾功能亢进的临床研究

目的研究肝硬化脾功能亢进患者行部分性脾动脉栓塞联合自体骨髓干细胞移植治疗的临床疗效。方法选择2010年6月~2012年5月38例肝硬化脾功能亢进患者行部分性脾动脉栓塞联合自体骨髓干细胞移植治疗,对比分析患者术后3个月血常规、PT等生化指标和脾脏大小及临床症状的改善情况。结果部分性脾动脉栓塞联合自体骨髓干细胞移植3个月后,患者血常规(RBC、WBC、PLT)显著上升,ALT、AST、PT明显改善,与治疗前相比差异显著,具有统计学意义(P<0.05),脾脏体积均有不同程度缩小,临床症状较术前明显改善。无一例严重并发症发生。结论部分性脾动脉栓塞联合自体骨髓干细胞移植治疗肝硬化脾功能亢进优势突出,疗效确切、满意,并发症少。

Application of partial spleen embolization and autologus bone marrow stem cell transplantation in treatment of liver cirrhosis with hypersplenism

Objective To study the effect of using Partial Spleen Embolization （PSE）and autologus bone marrow stem cell transplantation in the treatment of liver cirrhosis with hypersplenism. Methods The blood routine, PT, ALT and other indexes, the size of the spleen, clinical signs of the 38 cases patients of liver cirrhosis with hypersplenism before and 3 months after they received the mixed partial spleen embolization and autologus bone marrow stem cell transplantation were compared. Results The blood routine （RBC, WBC and PET）, ALT, AST and PT of the patients in follow-up 3 months after the treatment were significantly improved than those before the operation （P〈0.05）, the size of the spleen diminished to a varying degree, and the clinic symptoms obviously improved also. There were no severe complication. Conclusion It is effective and safe to apply PSE and autologus bone marrow stem transplantation in patients of liver cirrhosis with hypersplenism.
