踝关节骨折内固定术后两种镇痛效果比较

目的观察塞来昔布联合曲马多用于踝关节骨折内同定术后的镇痛效果。方法选择2009年12月～2011年3月行踝关节急诊切开复位内固定患者47例，将其分为两组：塞来昔布＋曲马多联合口服用药组（A组），共25例；单独应用曲马多组（B组），共22例。观察两组视觉模拟评分（VAS），有无恶心、呕吐等不良反应。结果A组术后VAS评分明显优于B组，差异有统计学意义（P〈0．05）。A组不良反应发生例数也低于B组。差异有统计学意义（P〈0．05）。结论术后塞来昔布联合应用曲马多对于踝关节骨折急诊手术患者有明显的镇痛作用。并可以减少使用传统镇痛药物及由此带来的副作用，提高术后康复效果，临床中值得推广使用。

Comparison of two analgesic efficacy after internal fixation of ankle fractures

Objective To observe the postoperative analgesic effect of Celecoxib combined with Tramadol applied to internal fixation of ankle fractures after operation. Methods 47 cases taking ankle joint emergency open reduction and internal fixation from December 2009 to March 2011 were chosen and divided into 2 groups： Celecoxib combined with Tramadol oral administration group （group A）, a total of 25 cases; Tramadol used alone group （group B）, a total of 22 cases. The visual analogue scale （VAS）, adverse reactions such as nausea, vomit were observed. Results The VAS scores after operation in group A were superior to those of group B, the differences were statistically significant （P 〈 0.05）. The cases of adverse reactions in group A were less than group B, there was a statistically significant difference （P 〈 0.05）. Conclusion Celecoxib combined with Tramadol has a significant postoperative analgesic effect in emergen- cy surgery for ankle fractures, can reduce side effects of the use of traditional analgesic drugs and can improve the postoperative effect of rehabilitation, which is worthy to be popularized in clinic.
