颅内动脉瘤破裂伴颅内血肿的急性期手术治疗

目的探讨颅内动脉瘤破裂伴颅内血肿的急性期手术治疗意义。方法对16例颅内动脉瘤破裂伴颅内血肿患者在48h内行手术治疗,术后1年以哥拉斯哥结果分级（GOS）标准对患者预后进行评价。结果16例患者显微手术夹闭成功。预后按GOS（哥拉斯哥结果分级）评价恢复良好7例,生活自理3例,重残2例,植物生存2例,死亡2例。结论对颅内动脉瘤破裂伴颅内血肿患者应积极行急性期手术治疗。

Surgery for Ruptured Intracranial Aneurysm Combined with Intracranial Hematoma in Acute Stage

Objective To investigate the surgical significance in acute stage for ruptured intracranial anemysm combined with intracranial hematoma. Methods Sixteen ruptured intracranial aneurysm patients with intracranial hematoma underwent microsurgical operation in 48 hours. After one year, Glasgow outcome scale（GOS） was conducted to evaluate the patients＇ prognosis. Results All the sixteen patients were operated successfully, based on GOS, good outcome was achieved in seven cases, moderate disability in three cases, severe disability in two cases, persistent vegetative state in two case, and two patients died. Conclusions The patients suffered from ruptured intracranial aneurysm combined with intraeranial hematoma should be operated in acute stage actively.
