急性缺血性视神经病变的眼底荧光血管造影

摘    要:目的 应用荧光素眼底血管造影的方法探讨缺血性视神经病变(FFA)的病理变化及对预后的影响。方法 回顾分析了38例42眼前部缺血性视神经病变(AION)及3例5眼后部缺血性视神经病变(PION)的临床资料。结果 38例42眼AION和3例5眼PION的FFA检查:臂一视网膜循环时间延长32眼,盘周脉络膜荧光充盈缺失12眼,视盘全部或部分荧光素充盈延迟或充盈缺损,并伴有相应的视野缺损,晚期视盘缺血呈强荧光或荧光素渗漏42眼,呈均匀一致弱荧光5眼。结论 缺血性视神经病变发病突然,PION比AION的视功能损害更加严重,治疗、预后欠佳。

Abstract:Objective To evaluate the pathology and prognosis of ischemic optic neuropathy with the examination of fundus fluorecein angiography(FFA).Methods The clinical dats of 38 cases (42eyes)with anterior ischemic optic neuropthy (AION) and 3 cases (5 eyes) posterior ischemic optic neuropthy (PION) were retrospectively analyzed.Results The data of FFA of AION 38 cases 42 eyes and PION 3 cases 5 eyes showed arm-retinal circulatory time of 32 eyes were delayed. Choroidal filling time of 12 eyes were absent near optic disc. All or a part of optic disc had postponed or absent fluorescence filling with the corresponding visual field defects. At the late stage of FFA high fluorescence or leakage were detected 42 eyes at the ischemic area of the optic disc, low fluorescence were detected 5 eyes.Conclusion There were significant differences comparing PION with AION, PION is not better than AION in treatment and prognosis.
