应用ROC曲线定量分析CtnI与CK-MB在心肌梗死诊断中的作用——应用ROC曲线验证cTnI的医学决定水平

目的　应用ROC曲线定量分析CtnI与CK -MB以探讨CtnI与CK -MB对心肌梗死的诊断价值 .方法　测定 5 0例心绞痛与 4 0例心肌梗死病人血清CtnI与CK -MB值 ,对CtnI、CK -MB进行ROC分析 .结果　用心绞痛组与心肌梗死组 (cTnI值 )进行比较 ,p<0 .0 0 1,用ROC曲线分析 ,CtnI的AUC =0 .98,CK -MB的AUC =0 .87.在cTnI =0 .5ug/l时 ,敏感性为 95 % ,特异性 96 % ,阳性结果预示值 =95 % ,准确度 =95 .6 % .结论　CtnI可以作为诊断心肌梗死的首选指标

To Compare cTnI and CKMB in the Diagnostic Value of Myocardial Infarction with ROC Curves Quantitative Analysis. --To investigate the cut-off point of cTnI with ROC curves quantitative analysis

Objective To compare cTnI and CKMB in the diagnostic value of myocardial infarction with ROC curves quantitative analysis. Methods We detected 50 patients with angina pectoris and 40 patients with myocardial infarction. T-examination and ROC curve analysis was used to analyze cTnI and CKMB among those patients. Results The median value of cTnI was significantly higher in the patients with myocardial infarction than in the patients with angina pectoris (p<0.001). The area under curve of cTnI was 0.98 whereas CKMB was 0.87. The cut-off point of cTnI (0.5 ug/L) showed sensitivity of 95%, specificity of 96%, accuracy of 95.6%, positive predictive value of 95%. Conclusion cTnI is confirmed to be the golden standard for detection of myocardial infarction.
