胃癌患者外周血DPD活性与5-FU化疗毒副反应关系的研究

目的 探讨胃癌患者外周血二氢嘧啶脱氢酶（DPD）活性与临床病理特征及5-FU为主化疗毒副反应的关系。方法 采用高效液相色谱技术检测66例胃癌患者化疗前外周血中二氢尿嘧啶（UH2）与尿嘧啶（U）的比值代表二氢嘧啶脱氢酶活性，所有患者行FOLFOX4方案化疗，化疗后评价毒副反应。结果 胃癌患者外周血DPD活性呈正态分布，与患者性别、年龄、发病部位、Borrmann分型、分化程度、浸润深度及肿瘤大小、是否有淋巴结转移等临床病理特征均无相关性，但与5-FU为主化疗引起的恶心呕吐、骨髓抑制、腹泻及口腔黏膜炎呈负相关。结论 胃癌患者外周血DPD活性检测可以作为预测5-FU为主化疗毒副反应的简便方法。

Study on the relationship between serum dihydropyrimidine dehydrogenase activity and toxicities of 5-FU in the gastric cancer patients' chemotherapy

Objective To explore the relationship between DPD activity and the clinicopathological characteristics, toxicities of 5-FU chemotherapy in gastric cancer patients.Methods The DPD activity in blood of 66 gastric cancer patients was detected by high performance liquid chromatography before chemotherapy.All of the patients received FOLFOX4 regimen. Toxicities of 5-FU were assessed.Results The DPD activity in blood was normally distributed in gastric cancer patients.The DPD activity had no correlation with the clinicopathological characteristics including sex,age,disease location,Borrmann type,histological grade,invasion depth,tumor size and lymph node metastasis.The 5-FU associated toxicities had a negative relationship with the DPD activity.Conclusions The detection of DPD activity in gastric cancer patients' blood can be a simple method to predict 5-FU-based chemotherapy toxicities.
