原发性肝癌患者术后肝内转移的介入治疗效果分析

目的探讨原发性肝癌患者术后肝内转移的介入治疗效果。方法选择32例原发性肝癌切除术后发生肝内转移的患者,采用Seldinger法经皮穿刺股动脉,插入4F RH导管至肝总动脉。先行全肝数字减影血管造影(DSA),根据造影结果显示的肿瘤血管,引入超滑导丝将4F RH导管超选至肝固有动脉或肝左、肝右动脉,经导管灌注5-氟尿嘧啶和奥沙利铂,并以超液态碘化油混合表阿霉素-盐酸吉西他滨(泽菲)作为化疗栓塞。多次进行经导管动脉化疗栓塞(TACE),每次TACE间隔4～6周。结果经过3～5个TACE疗程,达到完全缓解9例,部分缓解15例,好转1例,稳定1例,进展6例。结论TACE治疗原发性肝癌术后肝内转移疗效肯定,能延长患者存活期。

Study on the Interventional Therapy for Patients with Intrahepatic Metastasis after the Operation of Primary Liver Cancer

Objective To explore the effect of interventional therapy for patients with intrahepatic metastasis after the operation of primary liver cancer.Methods 32 cases with intrahepatic metastasis after the operation of primary liver cancer were selected.Seldinger intubation method was used to percutaneous femoral artery insertion,then tumor vessels were showed by digital subtraction angiography(DSA).Leading 4F RH catheter to proper hepatic artery,left or hepatic artery,fluorouracil and oxaliplatin were infused to catheter,and ultra liquid iodized oil mixed with epirubicin-gemcitabine as chemoembolization.Transcatheter hepatic arterial chemoembolization(TACE) was conducted many times.Results After three to five times,9 cases were complete remission,15 cases were partial remission,1 case improvement,1 case stabilization,6 cases progression.Conclusion The curative effect of TACE was definite,and can prolong the survival period.
