介入治疗用于妇产科疾病39例临床疗效观察

目的探讨介入治疗在妇产科临床应用的价值。方法对煤炭总医院2005--2013年收治的部分妇产科疾病如子宫肌瘤、产后大出血、宫颈妊娠、宫颈癌等采用介入治疗的39例临床资料进行回顾性分析，观察治疗效果，分析介入治疗应用于妇产科临床的可行性。结果妇科组15例中，行栓塞术的患者术中出血量、手术时间均少于未行栓塞术者，差异有统计学意义（P〈0．05）。产科组19例产后大出血患者中，除1例为中央型前置胎盘患者剖宫产术中发生产后大出血、DIC，行子宫切除术后仍有盆底渗血不止，行双侧髂内动脉栓塞术外，其余病例均在应用促进子宫收缩等措施无效后立即行子宫动脉栓塞，术后均收到了明显效果，避免了子宫切除的后果。宫颈妊娠及中央型前置胎盘难免晚期流产的病例均在计划手术或分娩前行双侧子宫动脉栓塞，明显减少了手术出血，成功避免了大出血的风险。结论介入治疗已广泛应用于多种妇产科疾病的治疗，优点是具有微创性、恢复快，疗效显著、不良反应少、易被患者接受，在妇产科领域应用前景广泛。

CLINICAL APPLICATION OF INTERVENTIONAL THERAPY IN OBSTETRICS AND GYNECOLOGY

Objective To study the clinical value of interventional therapy in gynecology and obstetrics. Methods The clinical data of thirty- nine cases of obstetrics and gynecology diseases such as uterine fi- broids, postpartum hemorrhage, cervical pregnancy, cervical cancer and with interventional treatment in author＇s hospital from 2005 to 2013 were retrospectively analyzed. The treatment effect was observed and the feasibility of interventional therapy applied in clinical obstetrics and gynecology was analyzed. Results 15 cases in gynaecology groups, i~traoperative blood and operating time in patients with embolism opera- tion were less than no embolism patiengs, the difference was statistically significant { P〈0.05）. 19 cases of postpartum hemorrhage patients in obstetric group, except 1 case for the central type placenta previa in postpartum hemorrhage and DIC in cesarean section, who still had a pelvic floor ooze blood more than after hysterectomy after the bilateral iliac artery embolization, the other cases were applied to uterine artery em- bolization after promote uterine contraction measures such as invalid immediately, and they showed a sig- nificant effect at postoperative and avoided a consequences of hysterectomy. Conclusions Interventional therapy has been widely used in the treatment of many diseases in obstetrics and gynecology with the advan- tages of minimally invasive, less adverse reaction, quick recovery, easier to be accepted by the patients.
