HLAKV联合治疗糖尿病合并肺结核的临床观察

目的:分析糖尿病合并肺结核者采用2HLAKV/10HLV方案治疗的临床效果及对糖尿病的影响。方法:2000年3月～2009年4月收治的28例糖尿病合并肺结核患者,采用2HLAKV/10HLV方案治疗,疗程中每月进行痰结核菌检查及血常规、尿常规、肝功、肾功的检查,满疗程的3、6、12个月摄胸片。结果:28例完成疗程时痰菌阴转率82.1%,随访24个月痰菌复发率14.3%,肝功异常率3.6%,肾功损害率7.1%,无一例因不良反应而停药。血糖理想控制者2例(空腹血糖<7.2 mmol/L),较好控制着24例(空腹血糖<8.3 mmol/L),不良控制者2例(空腹血糖>8.3 mmol/L)。结论:对糖尿病合并肺结核者采用2HLAKV/10HLV方案治疗,治疗效果满意,药物不良反应轻,对糖尿病影响小,此方案值得在临床中应用。

Clinical observation of HLAKV joint treatment of diabetic patients with tuberculosis

Objective To analyze the clinical effect of 2HLAKV/10LHV program on the patients who had diabetic with tuberculosis and the influence to diabetic mellitus.Method 28 cases of diabetes who with pulmonary tuberculosis from March 2000 to April 2009 treated with 2HLAKV/10LHV program.During treatment,patients monthly had sputum examination,blood routine,urine routine,liver function,renal function,3,6,12 months of the full course of chestg films.Results When 28 cases completed the treatment,sputumconversion rate was 82.1%,the relapse rate of 24months follow-up was 14.3%,rate of abnormal liver function was 3.6%,rate of abnormal renal function was 7.1%.no case had drug withdrawal because of side effects.Blood sugar control standard: ideal control was 2 cases(fasting glucose was less than 7.2 mmol/L),better control was 24 cases(fasting glucose was less than 8.3 mmol/L),bad control was 2 cases(fasting glucose was greater than 8.3 mmol/L).Conclusion Using 2HLAKV/10LHV gram to the patients who had diabetes with tuberculosis has satisfied effect,light side effects,and had light influence to the diabetes,so this program is worth in the clinical application
